Submit a Missing Person's Case

This is NOT an official missing persons report form. This form is only for requesting that a case be added to our database after the person has been reported as missing & the case has been accepted by law enforcement. You must be over 18 years of age to submit a case to our database using this form.

If you are a family member or friend of a person who has been missing for over 30 days, please use this form to submit their case for inclusion in our database. All submissions are reviewed and verified with law enforcement before publication. If a missing persons report was declined by law enforcement, please reach out to admin@mylovedoneismissing.com for further assistance prior to submitting this form. Thank you.
